CQC's assessment framework is organised around five key questions: is the service Safe, Effective, Caring, Responsive and Well-Led? Registration applicants can use the same headings to test whether their proposed systems form a complete and coherent service.
A strong mock interview does not treat the five questions as five slogans. It asks how people will experience care, what staff will do, what evidence leaders will review and what happens when something goes wrong.
Safe: preventing avoidable harm
Safe answers should explain how the service will identify risk, protect people from abuse, deploy competent staff and learn from incidents while respecting rights and independence.
- How will safeguarding concerns be recognised, recorded, escalated and learned from?
- How will staffing levels and competencies match changing needs?
- How will medicines, infection and environmental risks be controlled where relevant?
- How will incidents and near misses lead to verified improvement?
Effective: achieving good outcomes
Effective answers connect assessment, evidence-based practice, staff competence, partnership working and lawful consent to outcomes that matter to each person.
- How will needs be assessed and reviewed with the person?
- How will staff be trained, supervised and assessed as competent?
- How will consent and mental capacity be addressed for each decision?
- How will the service work with health and care partners when needs change?
Caring: dignity, compassion and involvement
Caring answers should show how people will be treated as individuals and involved in decisions, including those who communicate in different ways or need advocacy.
- How will staff learn what matters to each person?
- How will privacy, dignity, culture and protected characteristics be respected?
- How will people remain in control of decisions and information about their care?
- How will leaders recognise and address care that is task-focused or disrespectful?
Responsive: meeting individual and changing needs
Responsive answers explain how support will be personalised, accessible and adjusted when circumstances change, and how concerns will influence service development.
- How will care reflect preferences, routines, communication and equality needs?
- How will changes in need trigger review and coordinated action?
- How will complaints be easy to make and used for improvement?
- How will the service reduce barriers to access and participation?
Well-Led: oversight, culture and improvement
Well-Led answers should demonstrate that capable leaders will receive reliable information, create an open culture, make accountable decisions and verify that improvements work.
- How will the provider, nominated individual and registered manager maintain oversight?
- What information will show whether the service is meeting its aims and duties?
- How will staff and people speak up safely and influence decisions?
- How will audits, incidents, feedback and outcomes lead to sustained learning?
Run a five-question mock interview
- 1Choose one realistic scenario that touches all five key questions.
- 2Answer each question with actions, responsibilities, evidence and escalation.
- 3Ask follow-up questions about what happens when the planned process fails.
- 4Compare the answer with the application, policies and staffing arrangements.
- 5Record gaps under the correct key question with an owner and due date.
- 6Repeat the scenario after improvements and check that the answer is clearer and evidence-based.
