CQC says a provider registration assessment may include a Nominated Individual Discussion. The nominated individual should be able to speak authoritatively for the provider organisation about how the regulated activities will be supervised.

Preparation should focus on provider-level oversight rather than duplicating the registered manager's day-to-day role. These practice questions help you explain authority, assurance, escalation and accountability in your proposed organisation.

What the nominated individual should be ready to explain

  • The legal provider, its ownership and decision-making structure.
  • The regulated activities, locations and people the service intends to support.
  • Why you have the authority, competence and capacity to supervise management.
  • How your responsibilities differ from those of the registered manager.
  • How serious risks and performance concerns will be escalated to the provider.
  • How you will remain informed without taking over routine day-to-day management.

Practice questions about governance and oversight

  • What information will you receive about safeguarding, incidents, complaints and staffing?
  • How will you know that audit results are accurate and reflect people's experiences?
  • What will you do if a registered manager repeatedly misses improvement deadlines?
  • How will the provider decide whether resources are sufficient for safe growth?
  • How will provider-level risks, decisions and actions be documented?
  • How will you assure yourself that statutory notifications and duty-of-candour duties are considered?

  • How will you support and challenge the registered manager?
  • What happens when the manager is absent or the leadership arrangement changes?
  • How will staff and people using the service raise concerns directly with senior leaders?
  • How will the provider respond to whistleblowing or evidence of a closed culture?
  • How will you monitor leadership competence and succession planning?
  • How will you keep CQC informed when relevant registration details change?

Build a convincing evidence trail

  1. 1Map every regulated activity to its nominated individual and registered manager.
  2. 2Agree a governance calendar covering reports, meetings, audits and provider reviews.
  3. 3Define thresholds for immediate escalation and who can authorise urgent action.
  4. 4Use minutes and action logs to record challenge, decisions and completion evidence.
  5. 5Test assurance through conversations, observations, record sampling and feedback.
  6. 6Review whether improvement changed people's experience, not only whether an action closed.
A nominated individual should not rely only on a dashboard supplied by the service. Strong oversight includes proportionate checks that the information is complete, accurate and reflected in practice.

Practise the discussion effectively

Use the organisation's actual chart, statement of purpose, business plan and governance arrangements. Practise follow-up questions that ask what you would do when assurance is incomplete, leaders disagree or risks require additional resources.

  • Answer in the provider's voice and identify the authority you hold.
  • Give concise examples of challenge, escalation and verification.
  • Avoid presenting the registered manager as solely responsible for quality.
  • Check current CQC guidance where a role or notification requirement is uncertain.
