The documents needed for CQC registration depend on the legal provider, regulated activities, locations and type of care service. Everyone needs the main provider application form, while additional forms and supporting evidence vary by application.

Use the latest CQC form and its supporting-document instructions as your final authority. This guide helps you organise the evidence and spot the connections that often cause avoidable inconsistencies.

Core application documents

The exact form set changes according to your circumstances. Build a submission index from the current CQC application pack so nothing is missed.

  • The main new-provider registration application form.
  • Additional location forms where the main form does not provide enough space.
  • Additional nominated individual or partner sections where applicable.
  • A separate registered manager application for each proposed manager when required.
  • A complete statement of purpose covering the provider, aims, locations and managers.
  • Any service-specific supplementary sections identified by CQC.

Supporting evidence commonly requested

Supporting documents help CQC assess whether the proposed provider is fit and whether the service is likely to meet the relevant regulations. The required list is not identical for every service.

  • Business plan, financial forecasts or evidence of financial viability.
  • Organisational structure and clear lines of accountability.
  • Recruitment, induction, training and supervision arrangements.
  • Safeguarding, complaints, governance and quality-assurance policies.
  • Medicines, infection prevention, risk management and incident policies where relevant.
  • Insurance, premises, equipment or contractual evidence where requested.
  • CVs, qualifications and other fitness evidence for relevant leaders.

What makes a document registration-ready?

  • It names the correct provider and uses the same service details as the application.
  • It reflects the real client group, delivery model, staffing and regulated activities.
  • Roles are assigned to people or job titles that actually exist in the proposed structure.
  • The process is practical, lawful and capable of being followed from day one.
  • It has an owner, approval status, version number and realistic review date.
  • Linked documents do not contradict one another.

Create a controlled submission pack

  1. 1Create a document index with the CQC requirement, filename, owner, version and status.
  2. 2Use a consistent filename pattern, such as topic, provider, version and approval date.
  3. 3Cross-check legal names, addresses, locations, activities and manager details.
  4. 4Convert only final documents to the requested format and confirm they remain readable.
  5. 5Ask the nominated individual or responsible applicant to approve the complete pack.
  6. 6Store the exact submitted version securely so you can answer follow-up questions.

CQC registration document checklist: track what is still missing

Start a checklist with one row for each document in CQC's current instructions for your service. Give every row an owner and record whether it is missing, being prepared, awaiting review or ready to submit.

For example, a Statement of Purpose marked ready should have matching provider and location details, the correct manager information and an approved version. A file saved with the right name but unfinished content is still a gap.

  • Document and requirement: what is needed and which CQC instruction it addresses.
  • Responsible person and due date: who will prepare or check it, and by when.
  • Version and review: the copy checked against the rest of the application.
  • Submission status: send now or keep available for a later request, following CQC's instructions.
